CMS star ratings are genuinely useful, but they're a starting point, not a verdict: the health-inspection rating (from unannounced on-site surveys) is the most objective, staffing hours predict day-to-day care better than the headline star, and the quality-measure rating leans partly on what the home reports about itself. Ratings also lag real changes and update quarterly. The report tells you all of that, so you weight the numbers correctly and then trust your own visit.
